What does this PR do?

Part of #277.
Depends on #334 and #333 (this branch carries #333's commit; until they merge, only the last 3 commits are new here).

Codex CLI reads its configuration from $CODEX_HOME when that variable is set and ignores ~/.codex/. Symposium still wrote its global hooks to ~/.codex/hooks.json and its MCP servers to ~/.codex/config.toml, so for anyone who relocated their Codex config the hooks never ran and the servers never started, while the plugins from #334 already went to $CODEX_HOME.

Hooks and MCP servers now go to $CODEX_HOME too. Without the variable nothing changes.

Verified against Codex CLI 0.159.3 with CODEX_HOME set: Codex ran symposium's hooks and started its MCP server from the relocated directory, and nothing was written under ~/.codex/.
