Map typed package provenance to versioned package and extension labels. Keep raw URLs behind the policy boundary and share source selection.

What does this PR do?
This PR defines the telemetry-owned policy that maps typed package provenance onto the frozen version 1 public naming vocabularies. It emits no telemetry.
Package naming accepts only an exact Cargo coordinate from the canonical crates.io source. Source rejection precedes coordinate validation, so private registries, Git, path, workspace, and unknown sources retain the contract's specific unnamed reason rather than becoming invalid coordinates.
Extension naming shares one source decision across plugins, skills, and future plugin commands. Canonical crates.io and the exact recommendations repository are public; every other source remains unnamed. The allowlist follows shared source URLs rather than configured registry names, and raw provenance never enters a telemetry row.
Tests cover every package reason, malformed and wildcard coordinates, the Cargo ecosystem guard, exact URL matching, the recommendations package/extension asymmetry, the real path-backed user-plugins registry, and the Cargo classifier-to-policy boundary.

Questions for reviewers.
Are the version 1 URL allowlists, package-versus-extension asymmetry, and unnamed-reason precedence the right public identity boundary?
