changed the title
Agents were eagerly updating PRs after code changes if a prior message had requested a PR create/update. Rewrite rule 6 to make clear that each gt create/modify/submit requires a fresh explicit request in the current message — prior requests do not carry forward.

Hopefully improving upon (a) agents over eagerly updating a PR even when not asked, and (b) updating the current PR instead of creating a new one even when told to "make a PR".
