We previously did not correctly interpret so-called "flat" RadioButtonGroups. This resulted in wrong behavior in Document method `insert_pdf`. This fix introduces a new Page method `repair_flat_rbgs()` which converts Flat RBGs to standard RBGs with the canonical /Kids <-> /Parent relationship.

Document.insert_pdf().It can also be invoked independently via
page.repair_flat_rbgs().
