Codex OAuth requests failed with HTTP 400 when Chat Completions content parts were forwarded unchanged to Responses. Convert text, image, and file parts at the provider boundary, including assistant history.
Verified with existing OAuth: strings, text blocks, images, PDFs, and assistant history.
