Fix failure modes found in the scheduled strict FUSE Jepsen runs and address review findings:
Renew an HLC lease when its logical counter reaches the physical ceiling, so crash recovery can issue a commit timestamp instead of repeatedly failing with hlc: physical/logical ceiling exhausted.
Use the leader-applied timestamp for filesystem snapshots. A follower's local applied watermark can be ahead of the leader serving routed reads; taking their maximum allowed a stale session value to overwrite a newly admitted operation's pending and retained counts.
Preserve the local snapshot watermark when a timestamp-less coordinator is wrapped by CoordinatorForwarder, so existing inodes remain visible.
Advance HLC recovery ceilings beyond the committed and observed physical time, so recovery works when the ceiling is already ahead of wall time.
Return EAGAIN when filesystem contention retries are exhausted. This reports a retryable conflict to FUSE callers and lets quiescent final reads retry instead of failing with EBUSY.
The journal-accounting divergence was reproduced in rename and SQLite diagnostics. Both traces showed a session update reverting the immediately preceding admission's accounting.
Verification
go test -p 1 -timeout 15m ./... — passed on 9f72d2d.
Review follow-up on 114cd90: focused regression tests and full go test -p 1 ./kv ./internal/filesystem passed.
Risk
The earlier SQLite ACK-gap quota failure did not recur in the long instrumented run, but that single occurrence did not expose its first missing ACK. This PR does not change the gap quota or claim a separate ACK delivery defect is resolved.
The HLC lease recovery fix does not work if the existing ceiling is already ahead of the proposed value, for example after clock skew between nodes. Commit timestamp issuance can then keep failing until the wall clock catches up. A latent filesystem read-timestamp problem also exists for unusual coordinator wiring. Address the ceiling proposal before merging, or explicitly accept the limitation.

This PR touches files that the TLA+ safety spec has an anchor on (per docs/design/2026_05_28_implemented_tla_safety_spec.md §3),
so an AI review is requested below to verify the implementation has not drifted
from the model.
kv/hlc*.go — Next() must respect the HLC-4 preconditions (i)/(ii)/(iii) from the design doc: bounded skew, logical-counter handoff on leader change (strategy (c) Observe(MaxAppliedHLC)), and the commit-time ceiling fence (fail-closed when wall_now >= physicalCeiling). Any change to the bit layout (48/16), the CAS loop, or the ceiling getter/setter is in scope.
kv/coordinator.go, kv/sharded_coordinator.go — RunHLCLeaseRenewal, hlcRenewalInterval, hlcPhysicalWindowMs constants, and the new-term detection that calls Observe(fsm.MaxAppliedHLC()) (strategy (c)). Any change to renewal cadence, group selection, or fail-closed behaviour is in scope.
kv/transaction.go, kv/lock_resolver.go — OCC commit-ts assignment, lock-map encoding (key, lock_ts) -> start_ts, and the LockResolver action OCC-3 depends on. (M2 spec will land OCC-1..OCC-5; until then the spec doc §5.2 is the contract.)
kv/fsm.go — FSM apply of HLC lease entries (SetPhysicalCeiling), and any future MaxAppliedHLC() accessor that strategy (c) needs.
store/mvcc_store.go — version visibility, snapshot install, and the MVCC-1..MVCC-4 invariants (M3 scope).
If the change is correct but requires a spec update, edit tla/hlc/HLC.tla (or the corresponding M2..M5 module once landed) and the design doc in the same PR. The tla-check workflow runs the TLC model check on the same paths.
