fix(bootstrap): first-boot FK race + single-flight hardening - #1130
Conversation
… harden KV verification Cherry-picked and integrated from PR #1019 by @mmcintosh (Mark McIntosh). Core fix: the bootstrap ran all five seed steps in one Promise.all, so on a fresh DB a consumer INSERT (RBAC seed, plugin install) could win the race against its producer (document type registration) and hit FOREIGN KEY constraint failed. The per-step .catch() swallowed it, leaving roles/plugins unseeded, then the KV marker latched the broken state for 24h. Changes: - Two-phase seed: Phase A (document type producers) awaits before Phase B (document consumers) runs in parallel - runStep/bootstrapOk tracking: partial bootstrap never writes the KV marker - isSystemSeedPresent: KV fast-path now verifies specific seed rows (role-admin + core-auth) instead of a generic document_types COUNT(*) - Backoff after failure: 30s cooldown prevents re-running on every request - bootstrapCorePlugins returns failed plugin IDs so failures block the marker - Telemetry only fires on successful bootstrap runs Tests: - bootstrap-fk-ordering.test.ts: real SQLite with FK enforcement ON - bootstrap-resilience.test.ts: backoff, KV verification, plugin failure - Updated existing fast-path/single-flight/skip-order tests for new query Fixes #1017. Closes #1019. Co-Authored-By: Mark McIntosh <mmcintosh@infowall.com> Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>
…fast-path gaps 1. Phase A failure now aborts Phase B: if document type registration fails, consumers are skipped — they'd hit the same FK constraint this fix prevents. 2. _setBootstrapTimingForTests: removed staleMs parameter that silently overwrote waitMs (both targeted BOOTSTRAP_DEADLINE_MS). 3. Removed tautological !bootstrapInFlight guard from backoff check — bootstrapInFlight is always null at that point in the control flow. 4. KV fast-path now logs bootstrapCorePlugins() failures instead of discarding the return value silently. 5. KV fast-path clears lastBootstrapFailureAt on success, preventing stale failure timestamps from causing spurious backoffs. Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>

- Line 113: Update the `first` mock used by the `docTypeCount` cases so its
result supplies the role and auth seed counts consumed by `isSystemSeedPresent`,
not just the document count. Add explicit assertions for the fast-path decision,
verifying no KV deletion when seeds are present and marker-key deletion when
`docTypeCount` is zero; apply these checks to other cases in this file that rely
on `docTypeCount`.

The `first()` mock returned `{ n: docTypeCount }` but
`isSystemSeedPresent` reads `row.roles` and `row.auth`. This made the
fast-path seed check always return false regardless of docTypeCount,
masking regressions in the skip-vs-fast-path decision.
Return `{ n, roles, auth }` so docTypeCount controls the seed-present
outcome correctly, and assert that KV deletion only happens when seeds
are absent.

Summary
Cherry-picked and integrated from #1019 by @mmcintosh onto current
main.The original PR couldn't rebase cleanly —
bootstrap.tswas heavily refactored onmainsince the PR was opened (path-based short-circuits, host-keyed KV markers,BOOTSTRAP_SYNC_KV_KEYthrottle,verifySecurityConfigMemoized). This PR manually integrates the PR's value-add features into main's existing architecture.Core fix (issue #1017)
On a fresh DB's first boot,
bootstrapMiddlewareran all five seed steps in onePromise.all. Two steps createdocument_typesrows while two others insertdocumentsthat FK-reference them. When a consumer INSERT won the race, D1 threwFOREIGN KEY constraint failed— caught and swallowed — leaving RBAC roles and core plugins unseeded. The KV skip-marker was written anyway (24h TTL), locking the broken state in.Changes

The original PR's
waitUntil-based bootstrap architecture (where bootstrap runs in background and requests only wait on a bounded signal) was not integrated — main's inline bootstrap +settlesWithinsingle-flight mechanism serves the same purpose and is already battle-tested.Tests
