Separates the runtime-configuration and live Node Annotator test changes previously bundled into #55.
This draft preserves those changes for independent review: a suite-wide production runtime fixture, adjusted configuration-test setup, and narrower expectations for volatile Node Annotator clinical fields. None is required for the Langfuse runtime integration; #55 passed the local full suite without them.
Review whether suite-wide production configuration is appropriate and whether each removed assertion should instead become a dedicated provider contract check before accepting this change. These changes have not been independently revalidated in this draft.
