/model: one connected provider draws no provider heading, though the manual says every provider gets one #1685

Description

@AbirAbbas

What happened

On staging 494939749 (2026-09-30), with only the default provider connected (OpenRouter), /model opens on a table whose first rows are model ↓ then › aion-labs/aion-2.0 — no provider heading. commands.md ("/model — pick a model") says: "Models from connected providers sit under their provider's name as a dim heading, default provider first".

Replication

Deterministic (no model call). A fresh home whose profile has one model source (the default OpenRouter key), codeaf, /model, page up to the top.

Where it goes wrong

internal/tui3/palette.go (~2794) returns the ungrouped default list when fewer than two sources are connected, and groupBefore (~2029) draws no heading when Group is empty. That looks deliberate; if so the page should say that a single provider's models are listed without a heading.

Acceptance

The picker and commands.md agree for one connected provider and for two (a tui3 picker test for each, plus the manual gate).

Found by the 2026-09-30 staging re-check before v0.5.0 (lane 6, F-6.1).
