Fix broken module source links and dead external references The module tree was reorganized (hooked_domain -> hooked_origin, local_host -> exploits/local_host, and several renames), so every module page's Code link 404'd; point them at their current paths. Update the Burp, RuboCop, OWASP XSS and framesniffing references to live URLs, and unlink the retired ones (PhoneGap, the beef-console branch, and dead conference/blog/podcast links) keeping the text.
